#1 Before you contract for the donation services of your perfect egg donor, you must be evaluated by a physician, and be ready to proceed with an egg donation procedure.

#2 In order to start your search for your perfect egg donor, we need for you to fill out a "Recipient Preference Questionnaire". You can do this easily online. Simply click on "Recipient Preference Questionnaire" Pink Button at the very end of this page below, fill it out online, and return it to us by clicking on the "Submit Form" button at the end of the questionnaire. THIS IS THE FASTEST WAY FOR US TO SERVE YOU. Once we receive this questionnaire from you, we will start sending you profiles of egg donors that fit what you want, as described in your questionnaire. There is no charge for this service.

#3 After you have selected one of our screened egg donors, and your doctor says that you are ready to proceed with an egg donation procedure; you will then need to request a contract with EDCD for her donation services.

If you are unable to obtain a questionnaire by either of these methods, call us at 972-424-9369 and we will send you one immediately.

Once we receive your completed "Recipient Preference Questionnaire" you will be sent profiles at no charge, which correspond to your preferences indicated on your questionnaire. If you desire additional profiles, simply contact EDCD and request them. When you select your perfect donor, you will need to request a contract for her donation services. 1. You will be sent a contract to sign, have notarized and return.
2. You will also be sent a "Selection Sheet."
3. Once we receive the following (within the given time frame allowed):

a. your signed and notarized contract and
b. check or money order for the total fees indicated on the "EDCD Fee Schedule", and
c. your "Selection Sheet", your perfect donor will THEN be removed* from the list of available donors. We will immediately begin with cycle coordination and you could be on your way to having that precious child soon!  

Contracted recipients have priority on donor selection, in the order their preference was made known in writing to EDCD, by their returned "Selection Sheet".

Checks or money orders for less for the full amount will not be considered to meet contractual obligations, and therefore, will not remove a donor from the list of available donors.

 

Returned contracts that are not notarized are unacceptable.

*If your chosen donor has already been selected by someone else in the interim, she of course, will have been taken off the available donor list, and will not be available at that moment. If your donor is willing and able to go through another cycle, you can choose to wait for her. If not, you can choose another donor, or request additional donor profiles.
